<p><strong>THINK requires dedicated and compassionate Community Health Workers X12 (CHW) to join our team and play a pivotal role in promoting community health and well-being. The ideal candidate will be passionate about addressing important health issues, providing education and support to individuals and groups, and fostering strong connections between healthcare providers and community members.</strong></p>
<p><strong>The positions will be based within the Mangaung and Fezile Dabi Districts.</strong></p>

<p><strong>Duties and Responsibilities include but are not limited to : </strong></p>
<ul>
<li><strong>Coordinate and participate in community events, workshops, and health fairs to conduct health screenings, distribute educational materials, and raise awareness about key health issues.</strong></li>
<li><strong>Provide individual and group education sessions on topics such as nutrition, preventive care, chronic disease management, mental health, and healthy lifestyles.</strong></li>
<li><strong>Serve as a cultural and linguistic bridge between healthcare providers and community members, ensuring accessibility and cultural competence in healthcare services.</strong></li>
<li><strong>Conduct tracing of loss to follow-up TB patients, provide support to re-engage them in care, assist with TB screening activities, and support patient adherence to treatment regimens.</strong></li>
<li><strong>Conduct tracing of TB contacts, identify eligible patients for TB preventive therapy (TPT), and facilitate their access to preventive treatment.</strong></li>
<li><strong>Assist community members in navigating the healthcare system, including enrolling in health insurance programs, scheduling medical appointments, and accessing social services.</strong></li>
<li><strong>Connect individuals and families with appropriate social services, financial assistance programs, and mental health resources as needed.</strong></li>

<li><strong>Matric Certificate.</strong></li>
<li><strong>Ability to speak local languages.</strong></li>
<li><strong>Previous experience working with NGOs and familiarity with TB-related activities are desirable.</strong></li>
<li><strong>Proven track record of community engagement, mobilisation, and development within NGO settings or social programs.</strong></li>
<li><strong>Familiarity with the local community and prior exposure to the health sector are advantageous.</strong></li>
<li><strong>Ability to effectively communicate with diverse individuals and groups, including community members, healthcare providers, and stakeholders.</strong></li>
<li><strong>Sensitivity to cultural differences and a commitment to providing culturally competent care and services.</strong></li>
